5.15 Requests for Leaves of Absence Without Pay - Procedure

  1. Prior to requesting a leave of absence without pay, the employee should consult with the Personnel Office regarding his/her proposed leave to see what effect, if any, the leave will have on his/her benefits (tenure for probationary employees, retirement, health plans, MSA, etc.).
  2. Requests for leave of absence without pay should include the exact dates, if any, the employee will be on paid leave (vacation, personal holiday, holiday credit, etc.) as well as the exact dates the employee plans to be on leave without pay. All accumulated vacation, CTO, holiday credit and personal holiday time must be exhausted prior to the effective date of LOA/WOP.
  3. Routing procedure for requests for leaves of absence:
    1. Library Faculty.
      1. Request should be addressed to Unit Head who forwards to the appropriate Assistant University Librarian to AUL/Admin Services.
      2. If approved at all levels, AOA/Personnel forwards request along with Form 110 to Associate V.P. for Faculty Affairs office for final implementation. AUL/Admin Services informs employee his/her request has been forwarded to the Associate V.P. for Faculty Affairs with recommendation for approval. Associate V.P./F.A. office notifies employee when final approval is received.
    2. Support Staff.
      1. Request should be addressed to Unit Head who forwards to the appropriate Assistant University Librarian to University Librarian to AUL/Admin Services.
      2. If approved at all levels, AOA/Personnel forwards Form P105A to Personnel Office for final implementation.
      3. Requests shall be submitted 30 days prior to the intended effective date.
  4. All employees must complete a clearance card before beginning their leave of absence.
